Using the ASCP-NCOA Falls Risk Reduction Toolkit Part 2: A Collaborative Approach

Want to learn how to integrate falls prevention risk assessment and intervention into your practice? This program is designed for you! In the second session of this 2-part activity, participants will continue to use the ASCP-NCOA Falls Risk Reduction Toolkit to perform a comprehensive assessment of falls risk-inducing medications and medical conditions. Physical and occupational therapists will show how simple functional assessments can be integrated in the overall approach.

Before participating in the live program, it is highly recommended that attendees view the following two webinars that provide needed background on falls among older adults, national and state falls prevention efforts, medical conditions and medications as risk factors for falls, and toolkit components. Pharmacist Learning Objectives

At the completion of this application-based activity, participants will be able to:

  1. Explain the key roles of other disciplines in a multifactorial approach to falls prevention. 
  2. Identify four functional assessments pharmacists can use to assess a patient’s fall risk.
  3. Name three potential collaborative partners to contact to enhance falls prevention services.
