Using the ASCP-NCOA Falls Risk Reduction Toolkit Part 1: A Companion to CDC's STEADI Toolkit

Want to learn how to integrate falls prevention risk assessment and intervention into your practice? In this first of two sessions, attendees will be introduced to the American Society of Consultant Pharmacists–National Council on Aging (ASCP-NCOA) Falls Risk Reduction Toolkit, a complement to the Centers for Disease Control and Prevention’s Stopping Elderly Accidents, Deaths, and Injuries (STEADI) Initiative for Health Care Providers. 

Before participating in the live program, it is highly recommended that attendees view the following two webinars that provide needed background on falls among older adults, national and state falls prevention efforts, medical conditions and medications as risk factors for falls, and toolkit components. Pharmacist Learning Objectives

At the completion of this application-based activity, participants will be able to:

  1. Define the role of comprehensive falls risk assessment and intervention in their practice utilizing falls prevention toolkits from the Centers for Disease Control and Prevention’s Stopping Elderly Accidents, Deaths, and Injuries (STEADI) initiative and the American Society of Consultant Pharmacists–National Council on Aging.
  2. Identify common risk factors for falls in the senior population.
  3. Utilizing patient cases, develop individualized recommendations for fall prevention based on a medication review.
